Momentum eSports TV: PUBG Mobile Tournament

We are organizing a PUBG Mobile tournament of 72 teams, where each team consists of five (5) members (the fifth member will stand as a substitute and is OPTIONAL).

Please fill out the registration form and provide all necessary information and any valid ID (passport, driver's license, birth certificate, TIN, NBI clearance, school ID, etc.). Once submitted, we will review your registration form thoroughly.

*** For players under 18 years old, please make sure to provide a PARENTAL CONSENT. The consent must be signed by the parent/s or guardian and a valid ID of the parent/s or guardian must be attached. WE WILL NOT ALLOW PLAYERS TO JOIN THE TOURNAMENT IF THE REQUIREMENTS ARE NOT SUBMITTED.

FINALS: October 27, 2019
REGISTRATION FEE: ₱ 500.00

IMPORTANT NOTICE: The top 2 teams that will be competing for the championship will undergo drug testing as required by Games and Amusements Board (free) before playing.
